Welcome to SRC, an Aetna Company. A leading provider of health care and related group benefits. We look forward to servicing you with health care benefits for your AmeriCorps program. The documents in the panel on the left will provide you with information on the SRC AmeriCorps program. The Program Summary will give you an overview of the programs administration. The Plan 1 & 2 Comparison will help you decide on the benefit plan to offer to your members. After you have reviewed Plan 1 and Plan 2 and have decided which plan best meets your members needs and your budget allowance, you will need to complete the forms provided at the end of the attached materials. You will need to complete an AmeriCorps Program Data Form, which allows SRC to set your new group up into our database. This is done only one time; you will not need to do this again as your program year-ends and begins a new year. An AmeriCorps Health Care Roster Form is used to enroll your members for coverage, drop their coverage or make any changes (address, name, etc.). If a member does not need coverage because they have coverage elsewhere (parents, spouse, other) they will need to sign a Waiver Form. Once SRC has received back the required set-up information we will send you a complete package of all the necessary materials needed to administer your health care program. This package will contain an Administrative Manual with a copy of the Policy, Brochures, ID cards, Plan Information Summary, Claim Forms, Rosters, Waiver Forms and other required forms depending on the plan you select. You will also receive a username/password for access to SRC’s secured website for AmeriCorps programs. Thank you for allowing SRC the opportunity to share with you what our plans offer.
